Specific for Employed

Standard Visa Requirements

• Application form – Available from agent. Do not leave any blank space; write N/A if not applicable.
• Original passport – Must be valid for at least 6 months prior to travel date.
• Photocopy of passport bio page (pages 2–3).
• Photocopy of OECD country visas (USA, Schengen, Australia, Japan, Canada, etc.) issued within the past 5 years.
• Two (2) passport-size photos:
  • With collar
  • White background
  • No white-colored shirt or blouse
  • No accessories
  • Neutral facial expression
  • Do not staple
• Original Certificate of Employment with Leave of Absence, indicating:
  • Position
  • Date hired
  • Compensation
  • Office address
  • Office landline number (mobile numbers not allowed)
  • Email address
  • Duration of leave within the tour period
  Must be issued at least 3 months prior to application.
• Photocopy of Personal ITR (Income Tax Return – BIR Form 2316).
• Original Bank Certificate – Minimum balance of PHP 300,000.00.
• Original Bank Statement – Issued at least 3 months prior to application with at least 6 months transaction history.
• Travel Insurance – Minimum coverage of €30,000.
• Latest Original PSA Marriage Certificate (if applicable).
• Latest Original PSA Birth Certificate.
• Photocopy of Credit Card Statements and Credit Limits (if applicable).
• Additional supporting documents such as proof of properties (Land Title, Car Registration, etc.), if applicable.

Additional Documents for Parents / Relatives / Sponsors

• Photocopy of passport bio page or any government-issued ID.
• Original PSA / NSO Birth Certificate – Issued in the present year.
• Original Bank Certificate – Minimum balance of PHP 300,000.00.
• Original Bank Statement – At least 6 months transaction history.
• Photocopy of properties (Land Titles, Car Registration, etc.), if applicable.
• Photocopy of credit card statements and credit limits (if applicable).
• Notarized Affidavit of Support.

If Employed

• Original Certificate of Employment (COE) reflecting:
  • Position
  • Date hired
  • Compensation
  • Office address
  • Office landline number (mobile numbers not allowed)
  • Email address
  Must be issued at least 3 months prior to application.
• Photocopy of latest ITR (BIR Form 2316).

If Business Owner

• Photocopy of Business Registration (DTI or SEC) – Must be valid.
• Photocopy of Business Permit or Mayor’s Permit – Issued in the present year.
• Photocopy of latest ITR (BIR Form 1701 / 1702).

• Enclose all documents in a long folder with the applicant’s name written inside a plastic envelope.

• All requirements must be printed on colored, A4-sized bond paper.
