General Requirements
Visa application form duly completed and signed.
Current passport with validity of at least 6 months after the end of the planned travel and must have two
empty pages submit together with the Old Passports.
Original Passport (remove cover; new passports must be signed on page 2)
Photocopy of Bio Page of current passport.
Photocopies of all visas and immigration stamps for the last 5-10 years.
Two (2) passport sized photo with white background and not older 6 months.
Non-refundable visa application fee to be paid upon submission of visa application.
Proof of ties in the Philippines.
1. Proof of social security contributions (if relevant)
2. Proof of family ties in the Philippines (e.g., Original and Photocopy of birth certificate of applicant and
marriage certificate). These certificates should be issued by the Philippine Statistic Authority (PSA).
3. If widowed, original and photocopy of death certificate of spouse
4. Copy of real estate property – title –deed, if relevant

Bank certification, Bank books (should be authenticated by the issuing bank), personal bank statements
showing regular income (6 months), credit card statements or balance covering the last six months
Certificate of leave of absence (for Government Employees only)
Latest Income Tax Return (1701, 2316)
Photocopy of company ID
Photocopy of IBP or PRC license for professionals

- Photocopy of proof of company registration issued by the Department of Trade of Industry (DTI)/Securities Exchange Commission SEC registration of business (proof of registration and commercial activity of the company)

- Photocopy of Business Financial Statement (Bank Certificates, company accounts, etc.)
